Design holistic care plans across the entire clinical continuum, actively integrating the unique preferences and values of patients and their families to ensure personalized, respectful care. Advocate for resource allocation by identifying and breaking down systemic barriers, ensuring patients seamlessly access the critical services they need for recovery. Evaluate medical necessity through continuous, proactive patient assessments and chart reviews to guarantee each individual is receiving the precise level of care required. Champion post-discharge success by acting as a dedicated liaison between patients and post-acute services, ensuring a smooth, safe transition from hospital to home or specialized care. Optimize clinical and financial resources through strategic collaboration with payers and the care team, balancing top-tier patient advocacy with efficient healthcare utilization. What minimum requirements you'll need Licensure / Certification /
Registration:
Licensed Registered Nurse credentialed from the Wisconsin Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required. BLS Provider preferred. American Heart Association or American Red Cross accepted. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
Education:
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
