(RN) - Palliative Care Northern Wisconsin | Based out of Rhinelander Full Time (1.0 FTE, 80 hours every pay period) 8-hour Weekday Day shifts Compassion. Accountability. Collaboration. Foresight. We are seeking a compassionate, highly organized Registered Nurse (RN) to join our Palliative Care team serving Northern Wisconsin. This role supports both inpatient and outpatient palliative services across multiple hospital sites and the northern region , working closely with nurse practitioners, physicians, patients, families, and community partners to ensure coordinated, patient-centered care. Position Overview The Palliative Care RN plays a critical role in care coordination, clinical support, communication, scheduling, and tracking of palliative consults and follow-up services. This position requires excellent clinical judgment, strong communication skills, and comfort managing workflows across inpatient, outpatient, telehealth, and embedded clinic settings. Qualifications Active RN license in Wisconsin and Michigan (or ability to obtain) Experience in palliative care, hospice, oncology, or complex care coordination preferred Strong organizational, communication, and documentation skills Ability to work collaboratively across multiple sites and disciplines Comfort with EMR systems, Excel, telehealth platforms, and clinical workflows Why Join Our Team Meaningful work supporting patients and families during serious illness Collaborative, supportive, interdisciplinary palliative care environment Opportunity to impact care delivery across rural and regional settings Based in beautiful Northern Wisconsin, serving communities with compassion and excellence Inpatient Palliative Care Participate in daily huddles, review consult lists, and prioritize patients Coordinate consult scheduling and communication with patients and families Provide education to patients and families Manage inpatient calendars, consult tracking, documentation, and reporting Support telehealth visits and assist with exams as needed Provide NP/provider support, triage calls, and follow up with patients and families Coordinate outpatient palliative referrals Outpatient & Clinic Support Maintain provider schedules, embedded clinic scheduling, and referral tracking Coordinate appointments with patients, guardians, or POAs Perform patient check-in/out, insurance verification, and follow-up scheduling Prepare charts and patient materials Communicate with facilities, fax orders and notes, and ensure follow-through Triage incoming calls and referrals to ensure continuity of careWhy You'll Love It Here:
