Responsibilities:

Provide individualized nursing care to our residents, including medication administration, wound care, and treatment plans Develop and implement comprehensive care plans in collaboration with the interdisciplinary team Conduct initial and ongoing assessments to identify and address resident needs Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination Maintain accurate and up-to-date records of resident care and progress Collaborate with other team members to ensure a supportive and respectful work environment

Requirements:

Registered Nurse license in the State of Wisconsin Current CPR certification Strong clinical skills and knowledge of Skilled Nursing best practices Excellent communication and interpersonal skills Ability to work effectively in a fast-paced environment
