Job Description
MDS Coordinator (RN) We are seeking an experienced RN MDS Coordinator to join our team and assist with MDS oversight for two skilled nursing facilities in the area. The position will be primarily based at one facility, with occasional travel to the second location as needed. The ideal candidate will have strong
SNF/LTC MDS
experience, be comfortable working with the interdisciplinary team, and understand the importance of timely, accurate assessments and documentation.Responsibilities include:
Completing and submitting MDS assessments Managing assessment schedules and deadlines Supporting care plan development Assisting with PDPM accuracy and documentation Working closely with nursing, therapy, social services, and other departments Helping ensure compliance with applicable requirementsRequirements:
Active RN license Prior MDS experience in a skilled nursing or long-term care setting Strong organizational and communication skills Ability to travel between locations as needed We offer health insurance, dental, vision, PTO, and a 401(k) with match. Mileage reimbursement is available.Pay:
