SummaryThis position is primarily responsible for assisting in and providing safe, consistent, and appropriate patient care within the LPN scope of practice. The LPN serves as a patient advocate and focuses on assessing client needs, developing and implementing treatment plans under the guidance of the provider, and providing comprehensive support related to high emergency room utilization and potentially avoidable hospital use. This role requires strong communication and problem-solving skills, as well as the ability to manage a diverse range of responsibilities. The individual must demonstrate strong computer skills and the ability to work independently.

Specific Duties and ResponsibilitiesPerforms duties with the opportunity for independent judgment under general supervision.

Conducts telephone and face-to-face outreach to assigned patients to improve their understanding of their health and the measures SCH is taking to manage their conditions.

Identify patient needs and refer them to appropriate internal and external support services.

Provides education to patients on appropriate use of the Emergency Department and available alternatives.

Facilitates communication and coordinates services between providers and patients.

Coaches patients in effective self-management of chronic conditions.

Motivates patients to be active and engaged in their health and overall wellbeing.

To educate within scope of practice and problem solve with patients to identify and address specific goals to support health and behavior/lifestyle change using a variety of assessment tools under the supervision/directions of the provider.

Coordinates and monitors services, including comprehensive tracking of patients compliance in relation to care plan objectives.

Enters patient information into the Electronic Health Record (EHR).Schedules appointments, maintains records, and communicates with insurance companies.

Reinforces instructions provided by the healthcare provider and clinical staff.

Collaborates with healthcare team members to implement care plans.

Maintains patient confidentiality per SCH policies.

Attend staff and department meetings as needed.

Performs other duties as assigned.

QualificationsGraduate of an accredited practical nursing program.

Current WV LPN license.

Ability to work in a multidisciplinary team.

Knowledge of clinical procedures and protocols.

Strong communication and organizational skills.

Experience with Electronic Health Records.

Basic CPR certification required.

Previous LPN experience preferred.
