Job Summary:
The MDS Coordinator is responsible for managing and coordinating the Resident Assessment Instrument (RAI) process, ensuring compliance with federal and state regulations. This role involves completing accurate and timely Minimum Data Set (MDS) assessments, collaborating with interdisciplinary teams, and optimizing reimbursement through proper documentation and case mix reviews.
Key Responsibilities:
Coordinate and oversee the RAI/MDS process, ensuring compliance with CMS guidelines and facility policies. Accurately complete and submit MDS assessments, including OBRA and PPS assessments, within required timeframes. Review and validate resident medical records, care plans, and supporting documentation to ensure proper coding and accuracy. Collaborate with nursing, therapy, dietary, and social services teams to develop and update individualized care plans. Monitor changes in resident conditions and ensure appropriate assessment and documentation adjustments. Conduct case mix reviews and ensure proper reimbursement by verifying supporting documentation. Educate and train staff on MDS-related procedures, assessment tools, and documentation requirements. Participate in quality improvement initiatives, care plan meetings, and regulatory surveys. Ensure timely completion and submission of required reports and assessments.
Qualifications:
Education:
Graduate of an accredited nursing program (RN or LPN required).
Licensure:
Current, active RN or LPN license in the state.
Experience:
Minimum of 1 year of experience in MDS coordination, long-term care, or a related field preferred.
Knowledge:
Strong understanding of RAI process, MDS 3.0, care planning, and Medicare/Medicaid regulations.
Skills:
Excellent organizational, analytical, and communication skills with attention to detail.
Technical:
Proficiency in MDS software and electronic health records (EHR).
