  • Part-time We are looking for a compassionate, organized, and experienced Registered Nurse (RN) to join our team on a per diem basis.
In this rewarding role, you'll have the opportunity to make a meaningful impact by providing nursing assessments, care coordination, and clinical oversight for participants enrolled in West Virginia's Aged and Disabled Waiver (ADW) Program and other Home and Community-Based Services (HCBS) programs. The ideal candidate is passionate about helping individuals remain safe, independent, and comfortable in their homes and communities. Experience with Medicaid waiver services, person-centered care planning, and community-based healthcare is preferred. Essential Duties and Responsibilities :
Conduct initial and ongoing nursing assessments for ADW participants. Evaluate participant needs and develop individualized plans of care. Monitor participant health status and identify changes that may impact service needs. Collaborate with case managers, caregivers, physicians, and interdisciplinary team members to ensure quality care delivery. Review and maintain documentation in accordance with WV Medicaid, ADW, HCBS, and agency requirements. Provide clinical guidance and support to direct care staff as needed. Participate in quality assurance activities and compliance reviews. Ensure services are delivered in accordance with participant rights and person-centered care principles. Communicate effectively with participants, family members, representatives, and referral sources. Complete required documentation accurately and within established timeframes.
Requirements:
Current and unrestricted Registered Nurse (RN) license in the State of West Virginia. Minimum of one (1) year of nursing experience required; home care, case management, waiver program, or community health experience preferred. Knowledge of the West Virginia Aged and Disabled Waiver (ADW) Program and Medicaid Home and Community-Based Services. Strong assessment, documentation, and care coordination skills. Ability to work independently and manage a flexible schedule. Excellent verbal and written communication skills. Proficiency with electronic medical records and documentation systems. Valid driver's license, reliable transportation, and proof of automobile insurance. Each Home Instead franchise is independently owned and operated.
