Job Description
Conduct comprehensive assessments of patients' physical, emotional, and mental health. Develop personalized care plans based on patients' medical history, current condition, and specific needs. Administer medications, wound care, injections, and other treatments as prescribed by the physician. Monitor vital signs and overall patient health, adjusting care plans as necessary. Educate patients and their families on managing illnesses or conditions, such as diabetes, heart disease, or post-operative care. Collaborate with doctors, physical therapists, occupational therapists, and other healthcare professionals to ensure comprehensive care. Update physicians on patient progress and any changes in condition. Coordinate with social workers and other resources to address any additional patient needs, such as equipment or social support. Maintain accurate and timely records of patient visits, treatments, and progress. Document any changes in the patient's condition and communicate these to the appropriate healthcare team members. Provide instruction on medication management, diet, exercise, and other aspects of care. Offer guidance on safety measures, such as fall prevention and the use of assistive devices. Help families understand the care process and how they can support their loved ones. Be prepared to provide emergency care in case of a sudden change in the patient's condition. Educate the patient and family on when to seek emergency care or contact their healthcare provider. Advocate for the patient's needs within the healthcare system. Help patients navigate the healthcare system, including understanding their rights and accessing necessary resources.
