Full Job Description Job Description:
The Care Connect Screener supports individuals seeking mental health and/or substance abuse services. In this vital frontline role, you would be responsible for welcoming and engaging clients during the initial intake and registration process. The ideal candidate is passionate about working with diverse populations, demonstrates empathy and professionalism, and is committed to creating a supportive environment where clients feel heard and respected. Your role will involve conducting preliminary screenings, gathering essential client information, and helping connect individuals to the appropriate care pathways. This is a full-time, hourly position that works Monday-Friday, 8am-5pm. What you'll be doing: Answer telephone performs telephone registration. Obtain information on caller, i.e. demographic, insurance, problem/issue. Verify insurance information with third party payer sources. Complete registration paperwork via electronic medical record. Perform a general screening regarding severity of problems/issues. Perform Crisis Intervention / resolution as necessary. Consult with Crisis Therapist regarding immediate client needs. Collaborate with internal and/or external agencies regarding clients. Verify previous client's demographic information, update computer record. Schedule an intake appointment. Client Chart Maintenance, copying, filing, faxing, etc. Answer basic practice questions, maintain & update forms. Complete other duties as assigned.
