DUTIES, FUNCTIONS AND RESPONSIBILITIES
Provides mature professional judgment, utilize treatment modalities, exercise culturally sensitive therapy, case management and crisis intervention to reach independent conclusions in serving vulnerable patient populations with complicated health outcomes. Performs therapeutic skills in the evaluation, diagnosis and treatment of patients and their families who presents with a range of difficult health-related, psychosocial and environmental problems. Provides continuum of care by utilizing community-based resources, integrated treatment plan to patients and their families with the goal of returning to the baseline of their highest level of health. Prepares and gives interpretation of patient clinical reviews and psychosocial histories for the interdisciplinary healthcare team and community social service agencies for health outcome decisions. Performs initial assessment and evaluation of patients and families to identify treatment needs and barriers to growth and development. Develops treatment plans that address and specify outcomes, collaborates with interdisciplinary healthcare team on the psychosocial factors relating to illness, hospitalization, diagnosis and recommended treatment. Prepares patient referrals and coordinates services with community-based agencies and professionals (social services, school counselors, and behavioral health personnel) to help improve patient and their families' health outcomes. Participates in community-based agencies, workgroups or committees to address challenging and unmet healthcare needs while advocating for patients' healthcare needs, services and to address improvement of community resources. Makes professional decisions, planning and recommendations for patient treatment and discharge plans that include placement to a skilled nursing facility, group home, long-term care, or returning home, if indicated by the interdisciplinary healthcare team. Obtains data through chart reviews and other collateral information that is required to evaluate patients' health-related, psychosocial and environmental problems/needs Performs consultations and coordination with patients, family, community-based resources, and the interdisciplinary team. Participates in discharge planning and inpatient rotation for patients across the lifespan on the medical floor comprised of Multi-Service Unit (MSU), Intensive Care Unit (ICU), and Obstetrics (OB) Ward. Participates actively in program planning to raise the quality and comfort of care for frail and vulnerable patient populations. Performs other duties as assigned.
MANDATORY MINIMUM QUALIFICATIONS
Experience:
One (1) year of direct work experience as Medical Social Worker in a healthcare setting and/or community-based social services.
Education:
Master's Degree in
Social Work Licensure:
