Job Description

Responsibilities when joining the Haggai Healthcare team is first and foremost is showing up each day as your best self, working with purpose while having fun and Doing What is Right, Not What is Easy! Here are the actual job responsibilities for the
Intake Coordinator:
Coordinates initial home care referrals following state and federal guidelines Serves as a liaison between referral sources, patients/caregivers and Haggai Team Members Ensures that the patient has a smooth transition into the home health continuum of care Records all intake information and inputs into computer system and gets approval from Director of Clinical Services to proceed with referral/intake process
Verify:
Medicare eligibility; physician licensure/OIG clearance/NPI/PECOS; status of PCP at intake Track all referrals and conversions(admissions/non-admits) and report to leadership daily Coordinate initial staffing of referrals and ensure receipt of referral for care by all disciplines ordered Attain goals as set forth in annual performance evaluation Maintain strict confidentiality on patient, agency, and employee matters Handle a variable workload based on referral patterns and supports other roles
Pay:
$22.00 - $25.00 per hour
