Your Impact As the Home Health Intake Coordinator (LPN/RN) you will perform patient intake, referral coordination, and admissions processes. You will serve as the first point of contact for patients and referral partners, ensuring timely admissions, verifying insurance, coordinating care services, and delivering a seamless transition into home health.
You will:
Oversee all daily patient intake and referral operations Manage the full intake process, including insurance verification and authorization Ensure compliance with all state, federal, and payer requirements Coordinate timely patient admissions in accordance with company policies Build and maintain strong relationships with physicians, hospitals, and referral sources Ensure smooth patient transitions to home care, including care coordination and education Track and report referral data, trends, and performance metrics Promote and maintain a positive reputation in the community as a high-quality provider Identify opportunities to improve services and patient access to care Maintain knowledge of contracts, payer sources, and community resources Our Commitment to You Perform meaningful work focused on improving patient care and outcomes Work with a collaborative and mission-driven team that celebrates your success Ongoing professional development and training Our Requirements Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of Arizona Home Health experience a plus Experience in patient intake, admissions, or care coordination preferred Excellent communication, customer service, and relationship-building skills Knowledge of healthcare regulations and business operations Current CPR certification The employer for this position is stated in the job posting.
