We understand how fragile and disheartening this experience is on everyone, and we only want to ensure that our residents and their families are unconditionally nurtured and supported. At Care Plus Hospice, we are seeking a motivated and compassionate individual to work as an Intake Coordinator to service our patients in the Phoenix, AZ and surrounding areas . Job duties consist of, but are not limited to: Participates in coordination of hospice services and assigning staff members based on identified needs. Provides accurate and timely documentation of all consents to the Admission department. Support quality assurance and quality improvement Provides other administrative assistance and support duties as directed by the Director of Patient Care Services. Provides appropriate follow-up to all patients, families, and facility staff Complies with hospice policies and procedures, laws and regulations, standards of practice, and established work rules. Maintains accurate tracking systems to review documentation and ensure compliance.. Communicates with staff and outside agencies as directed. Facilitates communication between patients/families and other team members.
