Overview We re seeking an experienced Registered Nurse (RN) to lead our home health clinical team as Director of Patient Care Services. In this role, you ll drive high-quality patient care, ensure regulatory compliance, and support a collaborative, patient-centered environment. Responsibilities Lead and supervise all home health clinical operations and staff Ensure quality patient care in accordance with physician orders and care plans Maintain compliance with Medicare, Medi-Cal, and state regulations Oversee staff hiring, training, performance, and scheduling Drive performance improvement and ensure accurate clinical documentation Coordinate interdisciplinary care and serve as a key liaison with physicians Manage patient admissions, discharges, and overall care delivery Qualifications Active RN license 2+ years in home health 2+ years leadership experience Strong knowledge of Medicare/Medi-Cal regulations Excellent leadership, communication, and organizational skills Experience with EMR systems and Microsoft Office
