Mon-Fri, 8a-5p Minimum Qualifications Current LVN Certification required 1+ years experience with clinical intake, referral coordinating, home health or related healthcare experience Experience coordinating patient referrals, physician orders and scheduling workflows 2+ years of Cerner EMR experience Position Summary The LVN Clinical Intake Coordinator is responsible for coordinating home health referral intake, scheduling activities, and communication between patients, physicians, clinicians, and agency staff. This role supports efficient processing of physician orders, patient referrals, staffing coordination, and follow-up activities to ensure continuity of care and timely service delivery. Key Responsibilities Handle and coordinate incoming calls related to patients, physicians, referrals, and agency services Process and coordinate home health referrals, physician orders, and patient intake activities in a timely and accurate manner Facilitate communication between nursing, therapy, social services, aides, physicians, and other interdisciplinary team members regarding patient care needs and scheduling updates Assist with staffing and scheduling coordination while collaborating with leadership and clinical managers Provide follow-up support for hospitalized patients to ensure continuity of care and appropriate agency referrals upon discharge Communicate effectively with patients, families, providers, and internal departments regarding care coordination and service updates Maintain accurate documentation and ensure compliance with organizational policies and regulatory standards
