Full-time Job Summary We are seeking an experienced and detail-oriented and organized Intake Coordinator to join our home health team. This role is responsible for managing incoming patient referrals, verifying insurance eligibility, coordinating admissions, and ensuring timely start of care. Knowledge of fluent English and Armenian is a must! The ideal candidate is proactive, efficient, and experienced in home health intake processes. Responsibilities Manage and process high volume patient referrals from hospitals and physicians Perform detailed insurance verification Coordinate Start of Care (SOC) with clinical staff Enter patient information accurately into EMR systems Communicate with referral sources, patients, and field staff Ensure all required documentation is complete and compliant Track referrals and follow up to ensure timely admissions Maintain compliance with home health regulations and agency policies Qualifications Previous experience in home health intake required (2+ year preferred) Knowledge of Medicare guidelines and eligibility requirements Experience with EMR systems (WellSky/Kinnser, Axxess, etc.) Strong communication and organizational skills Ability to multitask and work in a fast-paced environment Effective verbal and written communication skills Bilingual (preferred ) Benefits $30 per house Supportive team environment Stable and growing agency Please submit your resume through Indeed or contact us at dianakay0808@gmail.com
