Job Description

This position is on-site only Imperial Valley Family Care Medical Group (
Location:
Dr. Palakodeti's Medical Clinic, 516 W. Aten Road, Ste. 1, Imperial Ca) is seeking a Medical Assistant (CMA) to support the Remote Patient Monitoring & Chronic Care Management Programs . Must be state-certified through the board of CCBMA, NHA, or AMCA agencies. Remote Patient Monitoring & Chronic Care Management Duties include:
Patient Assessment and Enrollment:
Assess patients' suitability for remote monitoring based on their medical history, condition, and clinical needs. Educate patients and caregivers on the benefits of remote monitoring, obtain informed consent, and enroll eligible patients in remote monitoring programs.
Data Monitoring and Analysis:
Monitor patients' health data and vital signs transmitted through remote monitoring devices in real-time. Review and analyze data trends, alerts, and abnormal readings to identify potential health concerns or changes in patient status. Respond promptly to critical alerts and escalate issues to healthcare providers as needed.
Patient Support and Communication:
Serve as a primary point of contact for patients enrolled in remote monitoring programs, providing ongoing support, guidance, and encouragement. Communicate regularly with patients to reinforce adherence to monitoring protocols, address questions or concerns, and promote engagement in self-management activities.
Documentation and Reporting:
Maintain accurate and up-to-date records of patient interactions, monitoring activities, and outcomes using electronic health record (EHR) systems or other documentation tools. Working knowledge of electronic medical records, ECW a plus. Please submit a complete resume with experience and education.
Competitive Hourly Rate:
$17.00 - $22.00 per hour and commensurate with experience. Adhere and comply with IVFCMG's required Executive Sub-Committees and all required regulatory Compliance and Quality measures. Comply and abide with Medical Home Responsibilities and Care Team roles and responsibilities. Participate and adhere to established Quality Improvement Activities or processes. Ensure Practice Visit Types are coded and used appropriately, such as ANN VISIT, SMDAY-URG and SMDAY-RTN, etc. Strive towards continuity of care for our patients, communication, medical records request, etc. Prepare treatment rooms for patient examinations, keeping the rooms neat and clean. Show patients to examination rooms and prepare them for the physician. Optimize patient flow, enabling the physician/mid-level to see more patients and conduct more visits. Interview patients to obtain medical information and measure their vital signs, weight, and height. Record patients' medical history, vital statistics, or information such as test results in medical records. Perform general office duties, such as greeting patients, log in patients' arrival, answering telephones, scheduling appointments, taking dictation, or completing insurance forms. Prepare and administer medications as directed by a physician. Convey clinical information on behalf of the physician/mid-level and follow clinical protocols when speaking with patients. Complete drug refill requests and provide prescription information to pharmacies. Maintain all medical inventory and order in a timely manner. Perform basic laboratory tests, EKG's, venipuncture, administer immunizations, collect and prepare lab specimens, remove sutures, and change dressings.
