WEEKEND POSITION
SATURDAY & SUNDAY 12PM - 5PM
We are seeking a dedicated and organized Intake Coordinator to join our thriving home health agency. As an Intake Coordinator, you will be the first point of contact for clients and their families, coordinating the intake process and ensuring a seamless transition into our care services. Your excellent communication skills and attention to detail will be crucial in maintaining accurate records and providing exceptional customer service.
Responsibilities:
Receive and process incoming referrals from various sources, including hospitals, clinics, and physician offices. Communicate effectively with patients, families, and healthcare providers to gather necessary information for intake assessments. Schedule and coordinate initial assessments and evaluations with our clinical team. Ensure accurate and complete documentation of patient information, medical history, insurance details, and care preferences. Collaborate with the billing department to verify insurance coverage and obtain necessary authorizations. Maintain a comprehensive understanding of our agency's services and eligibility criteria. Provide outstanding customer service, addressing inquiries and concerns from clients and their families. Keep accurate records of all communications and interactions related to the intake process.
Qualifications:
High school diploma or equivalent; associate's or bachelor's degree in a related field is preferred. Previous experience in a healthcare or home health setting is a plus. Excellent communication skills, both verbal and written. Strong organizational abilities and attention to detail. Proficiency in using computer systems and electronic health records.
