Admissions Coordinator Responsibilities :
Serve as the primary point of contact for individuals seeking admission to our recuperative care facility. Conduct initial screenings and assessments to determine eligibility for admission. Coordinate with healthcare providers, social service agencies, and other stakeholders to facilitate referrals and admissions. Collect and review necessary documentation, including medical records and insurance information, to complete the admission process. Communicate effectively with residents and their support networks to address questions and concerns and provide assistance as needed. Collaborate with the interdisciplinary team to develop individualized care plans and ensure continuity of care. Maintain accurate and up-to-date admission records and documentation in accordance with regulatory requirements.
Qualifications :
High school diploma or equivalent required; associate or bachelor's degree preferred. Compassionate and empathetic attitude towards individuals from diverse backgrounds, including the homeless population. Previous experience in a healthcare or customer service role preferred. Strong communication and interpersonal skills, with the ability to interact effectively with diverse populations. Excellent organizational skills and attention to detail. Ability to work independently and prioritize tasks in a fast-paced environment. Proficiency in computer skills, including Microsoft Office Suite and electronic medical records (EMR) systems. Compassionate and empathetic demeanor, with a commitment to providing patient-centered care.
