The Intake Coordinator is responsible for conducting comprehensive intake screenings, coordinating initial appointments, and ensuring a seamless transition to services. This position balances clinical sensitivity with operational efficiency to support timely access, appropriate level of care determination, and sustained engagement. Intake Coordinators document client background information identifies current concerns, symptoms, and functioning. They identify risk, conduct safety screening, and establish initial goals for care. The Intake Coordinator plays a critical role in shaping access to care and the overall client experience.Compensation Range:
$68,400- $73,000 per year
Essential Duties and Responsibilities:
Conduct initial screenings to gather client information, presenting concerns, and relevant clinical information. Must have core competencies that include clinical judgement and critical thinking, client centered communication and empathy, organization and attention to detail, ability to manage multiple priorities in a fast-paced environment, and collaboration and teamwork. Document detailed assessments and screenings in the electronic health record (EHR). Coordinate case assignment with the clinic multi-disciplinary team. Maintain comprehensive client referral tracking system. Assess client needs and determine appropriate level of care in alignment with organizational guidelines. Ensure all intake documentation meet regulatory, accreditation (CARF). Maintain confidentiality and compliance with HIPAA. Provide welcoming, supportive, and professional experience for all clients. Address barriers to care, including scheduling, transportation, child watch, or system navigation services. Contribute to process improvement initiatives related to intake, access, and client flow. Monitor and report on key metrics such as tie to intake, not-show rates, and conversion to ongoing care. Support outreach and engagement efforts as needed. Knowledge of local community resources and provide warm hand-offs for veterans and families needing external referrals. Able to work independently in a fast-paced outpatient clinical setting. Performs other duties as assigned to support clinic operations and organizational needs.Qualifications Qualifications:
Master's Degree in the behavioral health or social services field with active AMFT, LMFT, APCC, LPCC, ASW, or LCSW. Maintain license and/or certifications as required by state regulations. Post master's intake coordination and/or case management preferred. Experience working with Veteran and Military population highly preferred. Proficient with documenting within Electronic Health Record (EHR) Working knowledge of MS Word and Outlook. Experience and knowledge of de-escalation techniques. Knowledge of mental illness and mental health diagnoses. Knowledge of various treatment modalities and methods. Knowledge of community resources.Demonstrated Ability to:
Communicate clearly and effectively via oral or written means. Communicate appropriately with all levels of individuals. Make oral presentations and write clear detailed case notes. Present a professional and positive demeanor to staff, funders, officials, and the public. Remain calm and maintain self-control during difficult circumstances and emergencies. Represent the agency and respond professionally in all situations. Think analytically and evaluate the impact of treatment options and recommendations. Set work priorities and evaluate and create solutions to work related problems. Think analytically and conceptually on a long-term basis.Physical Requirements:
