" As the Director of Patient Care Services you will play a critical role in ensuring the well-being of our patients. Your leadership skills, nursing expertise, and knowledge of care coordination will contribute to the delivery of high-quality healthcare services. Responsibilities:
Attend and participate in IDG meetings and QAPI meetings Lead daily stand-up meetings, weekly stand-down meeting and morning call. Provide training for the RN's and LVN's to include disease specific training and procedure specific training. Supervision of the clinical support staff to include intake coordinator, volunteer coordinator, MSW's CHHA's, LVN's and RN's. Hold regular staff meetings with your clinical and clinical support staff to promote communication and growth within the organization. Manage mandatory reporting (HIS) Manage auditing clinical notes for compliance. Coordinate and oversee all direct and indirect patient services provided by clinical organization personnel. Provides guidance and counseling to coordinators, case managers and clinical support staff to assist them in continually improving aspects of hospice care services. Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues and expenses. Oversee the maintenance of patient clinical records, statistics reports and records for purposes of evaluation and reporting of organization activities. Plans and implements in-service and continuing education programs to meet education and training needs of the organization personnel at least quarterly. Assures compliance with all local, state and federal laws regarding licensure and certification of the organizational personnel and maintains compliance to the CHAP Home Care Standards. Stays informed about changes in the field of nursing and hospice care and shares this information with the appropriate organization personnel.
Qualifications:
DPCS EXPERIENCE WITH A HOSPICE GROUP A MUST - 3 YEARS
Bachelor's degree in Nursing, valid Registered Nurse License (RN) Must have at least 10 years of supervisory experience; 3 years as an RN preferred. Strong leadership and management skills, with the ability to effectively supervise a team of healthcare professionals In-depth knowledge of nursing practices, including patient assessment, monitoring, and medication administration Experience in developing and implementing care plans tailored to individual patient needs Proficiency in case management principles, including coordinating care in Hospice settings In-depth knowledge of Hospice care services Excellent organizational skills, with the ability to prioritize tasks and manage multiple responsibilities simultaneously Knowledge of medical records management and documentation requirements
