About the Company At AlphaCare Home Health, we believe patients deserve more — more time, more communication, and more care that actually changes outcomes. While many agencies use technology to increase visit volume, we use it to give clinicians more time with each patient. We are building a home health organization centered on clinical quality, meaningful patient relationships, multilingual community care, and strong operational support for the clinicians delivering care every day. About the Role The Intake Coordinator supports both patient intake operations and company-wide initiatives. This role coordinates referral intake, admission processing, and patient information management while also assisting with operational projects and organizational initiatives. Under general supervision, the coordinator performs preparation, data entry, verification, and editing of complex and confidential data while ensuring efficient intake operations and supporting strategic projects assigned by the CEO.
Qualifications/Educational Requirements:
High school diploma or GED with 2 years of experience directly related to the duties and responsibilities specified. Competent in computer programs such as home health software, Microsoft Word, and Excel as well the ability to use standard office equipment. Strong organizational, multitasking, and communication skills. Ability to maintain confidentiality of patient and company information. Ability to coordinate projects and support operational initiatives across departments.
Responsibilities/Essential Functions:
The person in this position must be able to perform the following essential job functions with or without reasonable accommodations. Handle incoming calls related to new patient intake and address inquiries from patients, patient representatives, and healthcare providers regarding patient information. Manage multiple tasks concurrently and prioritize responsibilities to ensure all critical aspects of the intake process are addressed promptly. Conduct outbound calls to patients, patient representatives, and healthcare providers to relay updates and share important information concerning patient care. Receive and route associated source paperwork to and from staff members and work with department representatives to facilitate efficient documentation flow. Create, maintain, and update data files, intake records, and logs while ensuring accuracy and compliance with internal policies. Extract and release information in strict compliance with established policies, procedures, and confidentiality regulations regarding patient records. Collaborate with team members and clinicians to facilitate accurate and timely communication of patient information. Communicate effectively, both orally and in writing, with staff members and management to ensure coordination of services and activities. Operate as a primary source of information on specialized data files and records for both internal and external stakeholders. Make administrative and procedural decisions and judgments on sensitive or confidential issues related to intake coordination. Support operational initiatives and special projects assigned by the CEO. Assist leadership with research, reporting, and coordination of cross-department initiatives. Track progress of assigned operational projects and report updates. Perform other related and assigned duties. Benefits Medical, Dental, Vision, and Life Insurance 401(k) with 5%
