This role includes insurance verification, authorization, data entry, and coordination of care, while serving as a key point of contact for patients, families, and referral sources. Key Responsibilities Oversee daily intake and referral operations to ensure timely patient admissions Verify insurance eligibility, authorizations, and reimbursement accuracy Coordinate patient transitions to home health services Provide direct oversight of patient education and preparation for home health, plan of care initiation, and coordination of care with multiple service providers. Maintain strong relationships with referral sources, case managers, and healthcare partners Track and report referral trends and intake metrics Support pricing discussions and payer communication as needed Qualifications Healthcare experience required, preferably in home health or hospice intake/referrals Strong knowledge of insurance verification, authorizations, and reimbursement processes Excellent communication, customer service, and relationship-building skills Ability to work independently with strong organization and multitasking skills Nursing background preferred, but not required The employer for this position is stated in the job posting.
