JOB SUMMARY
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery. The San Francisco Intake Services Program works with high-risk populations from a variety of backgrounds. Our programs are open and welcoming to adult participants, of any gender identity. We accept the majority of participants who seek Outpatient treatment, including walk-in's, referrals from other agencies and step-down transitional clients from residential treatment programs. The Patient Safety Navigator (PSN) will work within the Integrated Care Center and support client navigation to services within HealthRIGHT360 and the broader community. The PSN will be responsible for welcoming and assisting patients, and helping maintain a safe environment for clients, staff, and visitors. The PSN will also direct patient flow, escort patients to appointments, and provide Health

service information. The PSN will also use de-escalation techniques to address crisis and patient behavioral issues and serve as a first point of contact for new and returning clients.
KEY RESPONSIBILITIES
Client Services Welcomes and assists patients with navigating the clinical resources throughout the building. Coordinates care and patient flow to ensure timely provision of services, specifically the navigation between various health

programs. Provides overview of services to be rendered including but not limited to rapid testing for HIV and Hep C; tuberculosis screening; physical exams; addiction medicine services; resource center services and establishing primary medical and dental care. Escorts patients from any floor to clinic appointments. Provides excellent customer service and addressing any immediate needs of patients. Assists patients with required paperwork as needed for proper enrollment in services. Coordinates transportation for patients in residential programs. Safety Responsibilities Ensures and maintains a safe and orderly environment for clients, staff, and visitors. Directs and deflects traffic; de-escalates tense, high pressure, and emotionally charged situations; and fosters a safe, respectful, and welcoming environment. Maintains positive, respectful, non-confrontational behaviors always, even when observing, receiving, or de-escalating disrespectful or disorderly behaviors. Make regular rounds to monitor entrances, exits, offices, common areas, and building perimeter. Ensure a welcoming environment by clearing areas of crowds, debris, and out-of-place items. Work collaboratively with service providers and administrative staff to maintain an orderly environment. Administrative and Documentation Responsibilities Assists with data collection to monitor patient flow, works closely with the Admissions Department to help clients enter behavioral health programs. Performs property searches and personal inventory lists. Assists with incident reporting, surveillance video retrieval, and grievances. And perform other duties as assigned.
QUALIFICATIONS
Education, Certification, and Experience HS Diploma or GED required. First Aid and CPR certification. Community Health Worker or Mental Health related certificate strongly preferred. Preferably 2-4 years of 'lived experience' with the community behavioral health system and/or criminal justice system. Knowledge and Skills Familiarity with various supportive counseling strategies and wellness and recovery principles in working with clients with mental health, substance use, or co-occurring conditions. Knowledge of San Francisco community behavioral and public health systems. Understanding of social determinants of health and importance of whole person care Excellent verbal and written communication, planning, and time management skills. High degree of creative energy and the ability to adapt easily to change. Proficient in MS word, Excel, PowerPoint, and Internet applications. Team orientation and able to independently assume responsibility. Demonstrated ability to interface well with diverse, disadvantaged populations with multiple barriers to educational and vocational success, including low literacy, learning disabilities, physical disabilities, history of incarceration, and impaired cognitive functioning. Proven ability to build strong relationships with colleagues, community organizations, and the broader community. Willingness to work with clients in the community. Ability to respond quickly and use good judgment and unusual in emergency situations. Good attention to detail.
