Patient Care Specialist

Whole Health Pharmacy, located in Santa Ana, CA, strives to promote and achieve optimal therapeutic outcomes through collaborative Disease Therapy Management. Our goals include successfully improving the quality of life for each Patient, providing the highest standard of clinical services, reducing overall healthcare costs, and fostering relationships between Patients, Providers, and Payers. Seeking a bilingual Vietnamese-speaking Patient Care Specialist. The Santa Ana, CA based PCS should be highly motivated, team-oriented, and patient-focused. The PCS is responsible for providing customer service to patients referred to the pharmacy. Duties include, but are not limited to the following: Represent the pharmacy in a professional and courteous manner in all interactions with patients. Route patient referrals to the appropriate department or individual(s) in the pharmacy. Communicate patient information necessary to schedule services. Determine correct resolution to patient concerns; take corrective action, and monitor results. Prepare and secure Medical Necessity documents. Establish and maintain patient records. Communicate with intake specialist to ensure proper billing to insurance companies, contracts, other third party payers and patients for all products and services delivered; maintain accurate billing records. Scheduling initial assessments for patients and their first time medication. Coordinating with providers to deliver infusion medications. Coordinating home infusions for patients. Obtaining refill requests when appropriate. Preparing labels for the pharmacy technician to fill. Scheduling refills for patients through calls or SMS using the pharmacy management system and Salesforce. Perform other duties, as assigned by supervisor and PIC.
