Overture is hiring for a Part-time, Health Services Assistant the Longmont region.
Minimum Requirements:
High School diploma or equivalent required. Must have consistent access to reliable transportation. Must possess a valid Driver's License from the state in which you reside, maintain a clean driving record, and proof of insurance per company insurance guidelines. Must pass a motor vehicle record (MVR), background check, and other screening per Company requirements. Must be able to read, understand and communicate information and ideas verbally and in writing so others will understand. Basic computer skills. Consistent, reliable transportation for travel and errands. Basic knowledge of medical terminology, medications, and health care topics/issues preferred

Ever-evolving opportunities for growth Regular team-building and social events Essential Functions and Responsibilities (included, but not limited to): Maintain ongoing compliance with all required HR documentation and online/instructor-led trainings. Completes all trainings and documentation no later than the due dates given. Promote teamwork by effectively interfacing with a variety of individuals including health care professionals, families, individuals we serve, staff members from various service agencies, Host Home Providers, and Overture staff members. Ensure all necessary paperwork related to the individuals we serve are properly managed. This includes scanning and processing appointments in Therap, entering follow-up appointments, and scanning MARs after they have been reviewed. Assist Health Services Team in preparation of the HHP Compliance Spreadsheet by preparing monthly MARs, outstanding appointment reports, and health tracking reports. Update Health Services Master spreadsheet to track compliance with health appointments and protocols. Make necessary follow-up phone calls to medical facilities to obtain visit notes, lab results, or other needed paperwork. Make necessary phone calls to HHPs to request needed appointments, paperwork, or actions needed in their documentation. Work effectively with Residential team to problem-solve and carry out health support actions. Create Annual Orders document for the assigned region for review by HSC. After review, send to PCP office for signature. Follow up on outstanding medical/pharmacy bills received for individuals we serve to ensure that all insurance information is on record. Schedule and follow up appropriately on appoints for individuals we serve as assigned by Health Services Coordinator or Residential Program Manager. Participate and document meeting notes in agency staff meetings as assigned. Interface with Medicaid or Medicare programs as needed. Keeps all information about persons in services confidentiality in accordance with Overture policies and HIPAA regulations. Consult with Health Services Coordinator or Residential Program Manager when presented with problems outside of realm of expertise or to provide pertinent information regarding individuals we serve.
