Serves as a consultant in the field of Palliative Medicine, in both inpatient and outpatient settings. Serves as a hospice provider, offering clinical oversight on care of hospice patients, both inpatient. Assists Hospice Medical Director with documentation for certifications to obtain hospice benefits for patients.

7-8 consults per day Inpatient coverage only Some hospice care will be a part of this role
Summary:
Serves as a consultant in the field of Palliative and Hospice Medicine, in an outpatient setting.
Responsibilities:
Collaborates with colleagues in the management of patients, in outpatient settings Offers care to patients with critical and life-limiting illness, focused on assisting them to achieve medical treatment concordant with their goals. Offers expert symptom management to assist patients in achieving maximal quality of life, despite their illness. Builds effective relationships with referring physicians and care teams, strives to improve collaboration centered on the care of these patients. In both Palliative and Hospice settings, works with an interdisciplinary team, offering support to both patients and families. Works with colleagues to improve the quality of care offered patients
Additional Job Description:
Nurse Practitioner - Masters degree in Nursing, New Mexico Nurse Practitioner license, two (2) years working experience as a nurse practitioner. AANC or AANP certification Physician Assistant - Certified Physician Assistant, two (2) years working experience as a Certified Physician Assistant, and licensed by the New Mexico Board of Medical Examiners required.
