Compensation:

We're looking for the second person. At Soteria-360, you'll meet patients face-to-face, build relationships with healthcare professionals, explain a Medicare-covered support program, and help patients confidently enroll in services designed to keep them connected and supported after leaving the hospital. This is not a desk job. This is a r elationship-building role for someone who enjoys meeting people, earning trust, and making a meaningful impact in the lives of patients and families. About Soteria-360 Leaving the hospital can be overwhelming. Patients often have questions about what happens next, how they'll stay connected with their healthcare providers, and where they can turn for support once they're back home. Soteria-360 is a Medicare-covered transitional care program that combines physician oversight, telehealth, remote monitoring, and ongoing support to help patients remain connected and engaged during the critical weeks and months following hospitalization. What You'll Do Meet Patients Before Discharge Visit patients in hospitals and rehabilitation facilities Introduce and explain the Soteria-360 program Answer questions from patients and family members Build trust and help patients understand the benefits of the program Complete enrollment paperwork and required consents Help patients feel confident about participating Build Relationships Inside Healthcare Facilities Develop strong working relationships with physicians, nurses, case managers, discharge planners, and rehabilitation staff Become a trusted resource within partner healthcare facilities Collaborate with clinical teams to identify appropriate patients Maintain and strengthen professional relationships throughout your territory Guide Patients Through Onboarding The patients you enroll will also be the patients you help get started in the program after they return home.
You will:
Visit patients in their homes following discharge Deliver and set up simple monitoring equipment Connect devices to home Wi-Fi when needed Verify proper equipment operation Teach patients and caregivers how to use the equipment confidently Serve as a friendly and reassuring point of contact during onboarding No advanced technical experience is required. We provide training. Candidates should simply be comfortable with basic technology, including tablets, smartphones, Wi-Fi networks, televisions, and connected devices. Who Succeeds in This Role?
