Full Job Description Responsibilities:

Answers telephone calls directed to the intake department and prioritizes calls Obtains demographic information from referral sources and assigns the referral to an intake coordinator Enters intake data into the computerized system with 100% accuracy Serves as liaison for the Intake Coordinators working in the hospitals or nursing homes Notifies all appropriate departments of any changes related to the SOC Forwards insurance information to the Insurance Verifier, and ensures timely verification Organizes SOC information and assures placement of COC and ancillary services Performs all clerical functions of the intake department. Collaborates with Supervisors, and Coordinators. Consults with the supervisor when appropriate Represents Revival effectively and treats all customers courteously and in a non-discriminatory manner Performs other related duties as required Experienced with HHA Exchange Fluent in
