Job DescriptionJob Description Capstone Home Health is seeking a detail-oriented and organized Intake Coordinator to manage incoming referrals and coordinate the admission process for home health services. This role is essential in ensuring timely, accurate intake processing while delivering excellent communication with referral sources, patients, and internal staff.
