Job Description

We are a Medicare-certified home health agency dedicated to providing high-quality, compassionate care to patients in the comfort of their homes. Our team is committed to clinical excellence, compliance, and patient-centered care. We are seeking a detail-oriented and organized Intake Coordinator to manage and coordinate all new patient referrals and admissions. This role is critical in ensuring timely, accurate, and compliant intake processes in accordance with AHCA and Medicare guidelines. The ideal candidate should have clinical or medical knowledge, strong communication skills, and the ability to work in a fast-paced healthcare environment. An LPN is strongly preferred.
Key Responsibilities:
Manage incoming referrals from hospitals, physicians, and partners Review clinical documentation to ensure appropriate admission criteria are met Coordinate Start of Care (SOC) scheduling within 24-48 hours of referral Communicate with clinical staff, patients, and referral sources Ensure accurate and timely documentation in the EMR system Maintain compliance with AHCA, Medicare, and agency policies Responsible for the review of daily logs and notes from Case Managers to assure continuity of care, documentation of nursing process, appropriateness of intervention/action and timeliness of documentation. Support smooth transition from referral to admission.
Qualifications:
Clinical or medical background required Licensed Practical Nurse (LPN) preferred Previous experience in home health intake, admissions, or case coordination preferred Strong understanding of medical terminology and clinical documentation Excellent communication and organizational skills Ability to multitask and prioritize in a fast-paced environment Proficiency with EMR systems and basic computer applications
