Job Description

Location:
Korean Clinics in Duluth area.
Positions Available:
Patient Care Team Associate (2 opening)
Main Responsibilities:
The Patient Care Team is responsible for managing the RPM (Remote Patient Monitoring), CCM (Chronic Care Management), and APCM (Advanced Primary Care Management) programs at our clinic. The majority of our patients are Korean-speaking, and our EMR system is eClinicalWorks (eCW). This team plays a vital role in supporting primary care providers and ensuring quality patient care.
Key Responsibilities:
Program Enrollment & Coordination Identify eligible patients and educate them about the RPM, CCM, and APCM programs Handle patient enrollment, updates, and tracking within the EMR Document reasons for patient refusal and assist with re-engagement efforts Health Data Management & Monitoring Record and maintain vital signs and clinical data in the EMR (eCW) Review and interpret data from home monitoring devices (e.g., blood pressure monitors) Follow up with patients regularly, especially when abnormal values are detected Patient Communication & Education Deliver physician instructions and health education to patients Train patients on how to use monitoring equipment and log their health data Communicate via phone calls, text messages, and emails as needed Team Collaboration Work closely with front office staff, mid-level providers (NPs/PAs), and physicians Relay updates and patient requests to providers in a timely manner Assist with scheduling follow-up appointments and prescription refill requests Additional Support Duties Conduct pre-visit and post-visit patient check-ins as needed Address patient concerns or complaints and escalate when appropriate Prepare documentation for audits and compliance reviews for each program
Qualifications:
Bilingual (English + Korean) is must Proficiency in MS office and EMR Medical Background are preferred but not required (Training will be provided)

Full-time and Part-time positions available
Working Hours:
Monday - Friday, 9:00 AM - 5:00
