Job Title:
Intake Coordinator - Home Infusion Pharmacy Position Summary We are seeking a highly motivated and customer-focused Intake Coordinator to join our Home Infusion Pharmacy team. The Intake Coordinator serves as the first point of contact for patients, referral sources, healthcare providers, and insurance companies. This position requires excellent communication skills, strong attention to detail, and a consistently positive, professional attitude. The ideal candidate enjoys helping others, thrives in a fast-paced environment, and contributes to a culture of teamwork and exceptional patient care. Essential Responsibilities Receive and process new patient referrals for home infusion services. Communicate professionally and courteously with patients, physicians, hospitals, case managers, and referral sources. Gather and verify required patient information, clinical documentation, and insurance information. Coordinate with pharmacy, nursing, billing, and clinical staff to ensure timely initiation of therapy. Maintain accurate and complete patient records in accordance with company policies and regulatory requirements. Follow up on missing documentation and communicate updates to all stakeholders. Prioritize referrals and manage multiple tasks while meeting deadlines. Answer incoming calls and respond to inquiries in a professional and positive manner. Support patient satisfaction by providing compassionate and responsive customer service. Assist with other administrative and operational duties as assigned. Required Qualifications High school diploma or equivalent required. Minimum of one year of healthcare, pharmacy, medical office, or customer service experience preferred. Strong computer skills and proficiency with Microsoft Office applications. Excellent verbal and written communication skills. Ability to work independently while maintaining a team-oriented mindset. Strong organizational skills and attention to detail. Ability to maintain confidentiality and comply with HIPAA regulations.
