Full Job Description Position Overview:
Reliable Medical Services, Inc. is seeking a detail-oriented and organized Intake Coordinator to manage the patient intake process for our home health agency. This role serves as a key point of contact for referrals, ensuring timely processing, accurate documentation, and a smooth transition for patients into care services.
Key Responsibilities:
Receive, review, and process patient referrals from hospitals, physicians, and other sources Verify patient insurance coverage and eligibility Coordinate with clinical staff to ensure appropriate patient admissions Enter and maintain accurate patient information in the electronic medical record (EMR) system Communicate with patients, families, and referral sources regarding admission status Ensure all required documentation is completed and compliant with regulatory standards Schedule initial patient visits and coordinate care start dates Maintain strong relationships with referral partners and internal team members Track and report intake metrics and referral outcomes
Qualifications:
High school diploma or equivalent required; Associate's or Bachelor's degree preferred Minimum of 1 years of experience in healthcare intake, admissions, or administrative support (home health experience preferred) Knowledge of insurance verification processes, including Medicare/Medicaid Strong organizational and multitasking skills Excellent communication and customer service abilities Proficiency with EMR systems and Microsoft Office Ability to work in a fast-paced environment and meet deadlines
Preferred Qualifications:
