Job Description
The Center for Sleep Medicine , a progressive and AASM‑accredited, 100% employee‑owned sleep center, is seeking a detail‑oriented and compassionate Patient Care Coordinator to join us in our Mokena, IL office. This role supports patient outreach, appointment conversion, and Remote Patient Monitoring (RPM)/CPAP compliance workflows to ensure high‑quality patient care.

Key Responsibilities:
Contact patients to reschedule or convert appointments; document all outreach accurately. Educate patients on sleep medicine services and the importance of follow‑up care. Review
RPM/CPAP
usage reports and conduct follow‑up calls to support patient compliance. Identify barriers to adherence and help patients stay on track with treatment. Assist with basic operational tasks (scheduling support, documentation, referrals, etc.). Maintain excellent communication and customer service standards.
Skills & Qualifications:
Strong phone communication and interpersonal skills Proficient computer and data entry skills Ability to stay organized, accurate, and consistent with follow‑up Comfortable speaking with patients and explaining processes clearly
Education & Experience:
High school diploma required Healthcare or medical office experience preferred Familiarity with insurance or medical terminology helpful
RPM/CPAP
experience and bilingual skills (e.g., Spanish) a plus
