Day Shift Description:
Position Purpose Under the direction of the Cardiovascular Service Line Nurse Director and the CAD Program Coordinator, the Outpatient Cardiac Services Registered Nurse (RN) collaborates within a multidisciplinary team to deliver safe, effective, and patient-centered care. This role supports, guides, and educates patients and their families throughout the surgical process across the continuum of care. The RN assesses patient needs, develops and implements individualized care plans, and evaluates patient responses to achieve desired outcomes. The RN also serves as an advocate, assisting patients and families in navigating the complexities of the healthcare system. What you will do In this role, you will work as part of a collaborative outpatient cardiac team, ensuring high-quality care delivery and continuity of care.
Key responsibilities include:
Assess, plan, implement, and evaluate individualized patient care plans Educate patients and families on procedures, recovery expectations, and ongoing care needs Serve as a patient advocate and care navigator throughout the surgical continuum Communicate effectively with interdisciplinary team members, patients, and families Support quality improvement initiatives that enhance patient outcomes Utilize electronic medical record systems and clinical documentation tools Demonstrate teamwork through effective problem solving, communication, and conflict resolution
Minimum Qualifications Education:
Graduate of an accredited college or university affiliated nursing program. BSN preferred.
Licensure:
Current Licensure as a Registered Nurse (RN) in Connecticut.
Certification:
BLS (AHA/ARC).
Experience:
Minimum of 3 years of nursing experience within a cardiovascular surgical setting or related specialty. Experience working with a cardiovascular surgical team preferred.
Skills:
Basic computer proficiency required. Strong communication, organizational, and critical thinking skills.
Preferred Skills Education:
Bachelor of Science in Nursing (BSN) degree preferred.
Certification:
ACLS Experience:
Experience in outpatient cardiac services, care coordination, or patient navigation strongly preferred. Work Schedule Part Time
