Job Description

Intake Coordinator for Comprehensive Disease Management (CDM)
Full Time:
Monday through Friday, 8am - 5pm Wichita, KS office Phoenix Home Care & Hospice is a mission driven company. The Comprehensive Disease Management (CDM) Intake Specialist is responsible for managing and coordinating the care provided by our field staff members who work directly with clients in their homes. The role requires high levels of organization, attention to detail, excellent communication skills, and the ability to multitask in a fast-paced and rewarding environment. Benefits PTO (Paid Time Off) Multiple Major Medical Plans to choose from (Medical, Dental & Vision) Retirement Plan (401k, IRA) Life Insurance (Basic, Voluntary & AD&D) Paid Time Off (Vacation, Sick & Public Holidays) Short Term & Long-Term Disability Training & Development Responsibilities Receive and manage incoming referrals Interact with clients and/or client families to determine client needs Coordinate scheduling of initial visit with patient and CDM clinician Communicate details about new patients to clinicians and advocate for patient care Assist in tracking internal referrals amongst Phoenix service lines Attend weekly touchpoints with clinicians Internally build relationships for the furthering of serving clients Communicate effectively with CDM leadership team Requirements Possess strong time management and organizational skills with ability to multi-task Should be a detailed-oriented self-starter Must have exceptional customer service and strong phone etiquette Must have strong communication skills, both verbally and in writing Must be reliable with a strong work ethic. Must have basic knowledge of Microsoft Office (including MS Word, MS Excel, and MS Outlook) and ability to learn new basic databases for data entry Previous staffing or intake experience is a plus We're taking the journey with you, creating a New Beginning!
