Job Description

General Summary:
Maintain and coordinate systems, answer telephones, provide clerical and secretarial support to the Director of Inpatient Services and the interdisciplinary team. Records intake information and prepares for referrals and admissions.
Education:
  • High School Diploma or GED.
Experience:
  • Demonstrated ability to utilize various Microsoft applications.
  • Excellent grammar, keyboarding, and spelling skills.
  • 1-2 years previous experience preferably in a healthcare environment.
Licensure, Certification, Registration Required:
  • Valid drivers' license
  • Proof of current auto insurance
  • Working personal telephone (land or cell)
SUPPORT SERVICES COORDINATOR - HOSPICE CENTER POSITION COMPETENCIES 1.
Coordinates admissions to the Harry Hynes Memorial Hospice Center.
  • Receives admission information and initiates admission process to the Hospice Center.
  • Makes room/bed assignment in collaboration with the Hospice Center staff.
  • Maintains the admission log with patient name, patient room number, date and time of admission and date and time of discharge. 2. Maintains patient record in a manner that ensures accuracy, timeliness, safety, and confidentiality.
  • Assembles patient admission paperwork for chart and labels with patient name and medical record number.
  • Maintains an adequate supply of documentation forms.
  • Receives, sorts, and files all incoming documentation into correct patient chart per process.
  • Checks all documentation for completeness, identifying incorrect and/or missing items needed such as dates, signatures, names, etc., and notifying appropriate staff.
  • Faxes admission and death letters to physician offices.
  • Maintains a list of current census and provides it to the physician daily.
  • Reviews physician dictation for completeness.
  • Monitors admissions and updates admission log. Sends and receives paperwork per process. 3. Answers telephone and receives/screens visitors in a professional manner.
  • Greets all individuals with courtesy and consideration.
  • Answers telephone promptly and courteously, screens calls and uses judgement before releasing information to any person or agency.
  • Records messages accurately and relays to the appropriate person within fifteen (15 minutes), if other than routine.
  • Checks voice mail at frequent intervals, leaves voice mail messages, as needed, to relay appropriate information. 4. Performs duties/special projects directly related to area of responsibility.
  • Types correspondence, reports, and other materials in the proper format.
  • Assures a clean environment in staff area.
  • Assures that all furnishings and equipment are in good condition. Contacts the appropriate service vendor for maintenance, as needed.
  • Collaborates with Via Christi Saint Francis Hospital's (VCSF) housekeeping, as needed, to ensure a clean environment for patients/families and staff.
  • Collaborates with VCSF's laundry service to assure adequate supply of clean linens at all times.
