Job Description

PRIMARY RESPONSIBILITIES
Under the direct supervision of the Director of SGF & Crisis Services, the Intake Coordinator is responsible for completing the intake process for all SGF programs and IDD services, the waiver application process, scheduling and attending community outreach events, maintaining required documentation and billing concerning Access and Referral program and providing SGF/Crisis/PASRR case management.
Duties include:
Complete intake process for all new SGF referrals, including timely response to phone calls, facilitating meetings with individuals, families and guardians, collecting/completing required documentation and admissions into the EMR. Verify eligibility for services including the coordination of psychological evaluations and/or adaptive assessments when appropriate. Provide information to individuals and families concerning other community resources and make referrals when appropriate. Represent Pennyroyal Center IDD by attending and participating in and presenting at local and statewide meetings, conferences and events, including out of town/regional travel, weekends and evenings as needed. Serve as Pennyroyal IDD liaison with local schools, provider agencies, advocacy organizations (i.e.; local and state Arc chapters) etc. Work collaboratively with schools to facilitate a seamless transition for students from school to community supports. Provide SGF/Crisis/PASRR case management Assist families/individuals with applying for Medicaid waivers (MPW, SCL, HCB) and ensuring all individuals are Medicaid eligible. Responsible for proper and appropriate documentation of all supports provided in a timely manner. Provide crisis intervention, direct support and community resources to individuals as needed. Assist with MPW/SCL waiting list applications. (i.e.: update to request emergency SCL, check waiting list, etc.) Complete PASRR Level II evaluations as needed Perform other duties as assigned by supervisors.
MINIMUM QUALIFICATIONS
Minimum of Bachelor's degree in psychology, social work or related field. Minimum of five years' experience working with the IDD population. Must have a valid driver's license, reliable transportation and present proof of automobile insurance as required by the State of Kentucky and be able to travel throughout the region. Must pass a Criminal Record Check, a Child Abuse and Neglect Check, A Nurse Aide Abuse Registry check and a Sexual Offender Registry check. Must present proof of a recent (less than 1 year old) TB Skin test. Must be available 24 hours a day, 7 days a week to work a flexible schedule as required by program needs.
JOB REQUIREMENTS / EXPECTATIONS
Work in an office setting while conducting monthly home visits with SGF participants. Complete all required documentation in a timely manner, including case notes and person-centered care plans. Collect necessary documentation for waiver applications, utilize MWMA to complete and submit applications, and provide timely notifications to MPW recipients. Obtain PASRR certification training to conduct PASRR evaluations and complete required online risk mitigation training. Participate in meetings both via Zoom and in person, and perform all other duties as assigned by the supervisor.
