Job Description

Regional Patient Care Liaison (In-Market Leadership Role) Sweetwater Care | Skilled Nursing & Post-Acute Services Lead the Market. Build the Team. Drive the Results. Sweetwater Care is seeking a Regional Patient Care Liaison who doesn't lead from a desk you lead from the field. This role is for a proven performer who knows how to drive census, build referral pipelines, and elevate teams . You'll be responsible for both your own market production and coaching Patient Care Liaisons to perform at a higher level. If you're currently a top-performing liaison ready for the next step or a regional leader who misses being in the action this is your role. What You'll Own Drive census growth and market expansion across multiple Skilled Nursing Facilities in Ohio Maintain a personal presence in the field building and managing key referral relationships Lead from the front by producing results, not just reviewing them Coach and mentor Patient Care Liaisons to improve conversion, responsiveness, and strategy Create accountability around referral management, follow-up, and outcomes Identify market gaps and deploy targeted growth strategies that win This Is NOT a Desk Job You will be in hospitals, meeting with case managers, and owning relationships You will be actively driving business , not just directing it You will be expected to move fast, solve problems, and remove barriers What Makes This Role Different Player-Coach model you lead AND produce Real authority to influence strategy, performance, and market direction A company that values execution over meetings Opportunity to shape and grow a high-performing regional team What You Bring 2+ years in healthcare business development, hospital liaison, or post-acute sales Proven success in driving referrals, admissions, and market growth Experience coaching or mentoring others (formal or informal) Strong understanding of skilled nursing, payors, and hospital dynamics High accountability, urgency, and a results-first mindset What Success Looks Like Measurable census growth across your region Strong, consistent referral pipelines and conversion performance A team of liaisons who are engaged, coached, and outperforming expectations Being recognized in the market as a go-to partner and leader Ready to Step into Real Leadership?
