Intake Coordinator PRN

We offer behavioral healthcare services - psychiatry, addiction recovery, mental disorder treatment, etc. - that allow clients at all stages of recovery or rehabilitation to reclaim health and well-being.

Deliverables/Principal Results Expected:

Explain and sign admission paperwork, process, and copy driver's license/insurance information, and create/start the client treatment file. Contact Probation/Parole/Employer if needed for the client and assist in obtaining a letter for admission/discharge. Facilitate the admission/discharge process by communicating with all departments as needed. Maintain paperwork/notes/application statuses as needed for client stay/discharge needs per funding source requirements. Conduct level of care assessments on all clients. Complete aftercare goals with detox-only clients, set up aftercare, and complete discharge summaries. Other duties as assigned. Pay $15-$17

Licensure, Education, & Experience:

High school diploma or equivalent. Associate's or bachelor's degree in a human services-related field preferred.
