Qualifications The Patient Care Coordinator is responsible for the coordination of administrative and discharge planning for all patients. Essential Duties and Responsibilities Formulates Continuing Care Plan with patients. Facilitate Psycho-education weekly Assists patients with the completion of Short Term Disability and FMLA paperwork Initiates, explains, and completes the Referral and Intake Processes accurately. Ensures completion of Exit Surveys and delivery to appropriate staff for input. Observes all confidentiality regulations. Cordially greets and provides direction for patients and guests. Promptly responds to calls/mail in a professional, courteous, and helpful manner. Maintain filing system. Knowledge, Skills and Abilities Must have high level of interpersonal skills. Position continually requires demonstrated poise, professionalism, tact and diplomacy. Excellent organizational skills with the ability to prioritize workload and meet deadlines. Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint). Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization. Understands need for and maintains appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources and all other contacts. Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families and referral sources. Education and Experience Bachelor's degree in psychology, social work, or related field preferred. Minimum 2 years' experience in a mental health/healthcare field. Insurance verification and/or insurance billing experience are a plus.
