Be the Best Hospice Inpatient Visit You Can BeIf you meet these qualifications, we would love to meet you:

Maintains an active license and DEA in the state where care is being provided.

Willing to make hospital inpatient visits one week per month (Tuesday - Monday schedule)
Job Requirements:
Develop and maintain Plan of Care for each inpatient, including discharge planning discussions with care team, patients, and familiesParticipate in prognosis evaluation and certification/recertificationSign death certificates as neededConduct daily patient rounds and set consistent rounding schedules with interdisciplinary teamEnsure proper documentation of eligibility for inpatient careProvide education and training to interdisciplinary team and new staffProvide after-hours call coverage on days with inpatient visitsWhy AccentCare?
