Job Description

Intake Coordinator, RN or SW Region VII Area Agency on Aging/Sunrise PACE is accepting applications for a highly motivated Intake Coordinator, for Sunrise PACE. The PACE center and job location is in Fort Gratiot, MI (St. Clair County). We believe in advocating, planning, developing, and supporting a comprehensive system of quality care and services designed to achieve the optimum level of health, well-being, and independence of people as they age. Purpose of the position Responsible for the evaluation and coordination of new enrollees to the program; includes intake and marketing activities. Works as a team with intake partner to assess potential enrollees, provide education to enrollees and families about the program, and assist with the enrollment process.
Key Responsibilities:
Evaluates potential enrollees to the program to determine participant needs and eligibility for enrollment. Assessments completed in the homes of the potential enrollee. Coordinates all level of care decisions with State Health and Human Services personnel. Participates in Interdisciplinary Team, management team and other related meetings and activities. Participates in marketing activities to recruit potential new participants. Completes appropriate documentation related to participant assessments as required. Maintains statistical data required. Requirements for this position Must be able to work effectively in a team environment. Must possess excellent communication, interpersonal and conflict resolution skills. Knowledgeable of Medicare and Medicaid preferred. Ability to stay organized within busy work environment. Establish and maintain effective interpersonal relationships with all levels of staff, other departments, agencies, members, providers and the general public. Communicate effectively, both verbally and in writing, with individuals from varying cultural backgrounds Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact. Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse. RN, BSW or MSW licensure Minimum of 1 year experience in community health services preferred Experience working with a frail or elderly population or, in the absence of such experience, to receive appropriate training from the Sunrise PACE on working with a frail or elderly population upon hiring. Physical demands:
Employee must be able to sit for extended periods of time. Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for daily interactions with staff and elderly participants.
Work Environment:
