Home Connection Therapy is an In-Home / Mobile Outpatient Therapy service where services are provided in client's private homes, Independent, and Assisted Living Facilities. Currently looking for a PT to conduct evaluations and the following treatment sessions (on average 2x/week per patient). HCT is a small group of experienced therapists striving to provide a higher quality of individualized care in our client's own environment to inspire the highest outcomes. We help adults of all ages live their life to the fullest while maintaining independence in their home environment. Travel radius is negotiable/flexible around Grand Rapids and surrounding areas, some patients are on the same walkable campus. You make your own schedule with adjustments based on client preferences. HCT values personal time and time spent with family, there are no mandatory weekends, holidays, or evenings. Take time off for vacations as needed. No productivity requirements, easy documentation (less compared to traditional HH)! Currently an Independent Contractor position with the potential for growth.
