Job Description

University Home Care Inc is seeking a compassionate and organized Hospice Coordinator to join our team in Livonia, MI. The Hospice Coordinator will play a critical role in providing support and coordination for patients receiving hospice care, ensuring a smooth and compassionate experience for patients and their families. Responsibilities Coordinate hospice care services between patients, families, healthcare providers, and community resources. Develop and maintain comprehensive care plans tailored to each patient's needs. Serve as a primary contact for patients and families to address questions, concerns, and provide emotional support. Collaborate with interdisciplinary teams to promote the highest quality end-of-life care. Arrange and schedule visits by hospice care professionals and community service providers. Document and maintain accurate records in compliance with company policies and regulatory requirements. Participate in staff meetings and training sessions to stay updated on hospice care best practices. Requirements Prior experience in hospice care coordination at least a year or related healthcare field preferred. Strong organizational and communication skills. Ability to provide compassionate support to patients and families. Knowledge of hospice care regulations, procedures, and documentation standards. Proficient with electronic medical records (EMR) and Microsoft Office Suite. High school diploma or equivalent required; additional healthcare-related certification is a plus.
