Full-Time If you are a Speech Language Pathologist who enjoys meaningful patient interaction and wants the flexibility to practice outside of a clinic or facility, home care offers one of the most rewarding settings in healthcare. At Legacy Home Care and Home Health Care, Inc., our Speech Language Pathologists provide one-on-one care in the home , helping patients improve communication, cognition, and swallowing while remaining in the environment where they feel most comfortable. This full-time field role allows you to work across assigned territories, giving you variety in your caseload while still maintaining a manageable schedule. You will have the independence to make clinical decisions, the support of an experienced therapy team, and the opportunity to see the real-life impact of your work every day. As a Speech Language Pathologist in home care, you will evaluate patients in their home environment and develop treatment plans that improve functional communication, cognitive skills, and swallowing safety. This setting allows you to focus on real-world outcomes and provide care that directly improves daily life. Speech Language Pathologist | SLP Responsibilities Provide in-home speech therapy evaluations and treatment visits. Assess speech, language, cognition, and swallowing function. Develop and implement individualized plans of care. Provide communication and cognitive training. Evaluate swallowing safety and feeding techniques. Educate patients and caregivers on strategies for success at home. Collaborate with nurses, therapists, and physicians to coordinate care. Complete accurate and timely documentation in EMR. Speech Language Pathologist | SLP Qualifications Active Minnesota Speech Language Pathologist license. Valid driver's license and reliable vehicle. Ability to work independently in a field-based setting. EMR experience preferred. Strong patient engagement and caregiver education skills.
