As a Speech Language Pathologist in home care, you will evaluate patients in their home environment and develop treatment plans that improve functional communication, cognitive skills, and swallowing safety. Speech Language Pathologist | SLP Responsibilities Provide in-home speech therapy evaluations and treatment visits. Assess speech, language, cognition, and swallowing function. Develop and implement individualized plans of care. Provide communication and cognitive training. Evaluate swallowing safety and feeding techniques. Educate patients and caregivers on strategies for success at home. Collaborate with nurses, therapists, and physicians to coordinate care. Complete accurate and timely documentation in EMR. Speech Language Pathologist | SLP Qualifications Active Minnesota Speech Language Pathologist license. Valid driver's license and reliable vehicle. Ability to work independently in a field-based setting. EMR experience preferred. Strong patient engagement and caregiver education skills.
