This position combines Admissions , Case Management Support , Patient Visits , and Clinical Coverage into one dynamic role. A Role Designed for Nurses Who Want More. Many hospice organizations hire nurses into a single function. You are either an admissions nurse, a case manager, or a visit nurse. This role offers something different. As a Hospice RN Resource Nurse , you'll become a trusted clinical partner who can step into a variety of situations where your expertise is needed most. One day you may be helping a family navigate the hospice admission process. Another day you may be supporting symptom management visits, assisting with case management coverage, or responding to changing patient needs. Admissions & Start of Care Complete hospice eligibility assessments and admission visits. Provide patient and family education regarding hospice philosophy and services. Obtain consents and complete admission documentation. Perform medication reconciliation and initial care planning. Facilitate seamless transitions onto hospice services. Case Management Support Assist with ongoing patient care planning and coordination. Support symptom management and clinical interventions. Collaborate with patients, families, and interdisciplinary team members. Provide caregiver education and ongoing support. Patient Visits & Clinical Coverage Conduct routine and PRN patient visits throughout the service area. Respond to urgent patient and family needs. Provide coverage support during vacations, leaves, and staffing transitions. Assist with changes in condition and evolving care needs. Interdisciplinary Team Collaboration Partner closely with nurses, social workers, chaplains, hospice aides, providers, and leadership. Participate in care planning and team communication. Support patient-centered goals of care while ensuring continuity across disciplines. Documentation & Compliance Maintain accurate and timely EMR documentation. Ensure compliance with Medicare hospice regulations and agency standards. Complete required clinical documentation efficiently and thoroughly. What You'll Bring. Active Minnesota Registered Nurse (RN) License. Hospice experience preferred. Strong consideration given to nurses with backgrounds in
ICU , ER
, Oncology , Palliative Care , Critical Care , or other high-acuity clinical settings. Excellent communication and patient education skills. Comfort working independently in a field-based role. Strong critical thinking, prioritization, and clinical decision-making abilities. Adaptability and confidence in changing situations.
