What You'll Do:

Provide hospice nursing visits across homes, assisted living, and memory care settings. Deliver pain and symptom management, wound care, and skilled nursing assessments. Support patients and families through end-of-life transitions with compassion and education. Communicate confidently about care plans and hospice expectations. Work closely with an interdisciplinary team to provide aligned patient care. Maintain accurate documentation supporting continuity and quality outcomes. Participate in an on-call rotation supporting patient needs and continuity of care.

What We're Looking For:

Active Minnesota Registered Nurse license. Experience in hospice, ICU, oncology, palliative care, or end-of-life nursing preferred. Strong emotional intelligence and ability to support patients and families. Confident communication skills and strong clinical judgment. Comfortable working independently in a field-based role. Reliable transportation and willingness to travel throughout the Greater Twin Cities Metro Area.
