As an Occupational Therapist in home care, you will have the opportunity to practice at the top of your license while helping patients remain safe and independent in their homes. This includes evaluating daily living skills, improving fine motor coordination, completing home safety assessments, and recommending adaptive equipment that allows patients to live with confidence. Occupational Therapist | OTR/L Responsibilities Provide in-home occupational therapy evaluations and treatment services. Assess patient functionality, safety, and independence within the home. Develop individualized care plans with measurable goals. Implement therapeutic interventions to improve daily living skills. Recommend adaptive equipment and home modifications. Collaborate with nurses, therapists, and physicians to coordinate care. Complete documentation in EMR accurately and on time. Occupational Therapist | OTR/L Qualifications Current Minnesota Occupational Therapist license. Valid driver's license and reliable vehicle. Ability to work independently in a field-based setting.
